Melted In-line fuse?

I would suspect that one end of the fuse wasn't making a good connection to the holder. The added resistance at this point would create a hot point which would melt the case. Either there was some corrosion or it wasn't secured tightly.

The only other explanation is that the fuse was a higher rating than the holder, otherwise it would have blown and not overheated.
