Line driver fails suddenly

Hi everyone,

So I attached a picture of the wiring I did in my car and everything was going fine for a whole year.
But then about 2 weeks ago my line driver failed me and the red light on top was no longer turning. I checked with a multimeter and my wiring is live and firing 12v.

So because I was no longer on the warranty I had to buy another one. 2 days ago I received my new line driver which I just plugged and played with the old wiring since it's the exact same.
It worked like a charm and the red light on top was on also!
However, it failed me the very next day..

What would be your best guesses?

The line driver model is this one:
