Lanzar Opti4000d low output voltage

I built a box for a guy that's running a lanzar Opti4000d, I'm having some issues with it. I went to set gains with a dmm and the most I can get is around 30v with the gain maxed, then finally went into protect mode. Since he just put in a used HU we thought maybe something was wrong with the rca's, so I tested it in my ride (does the same thing). Took the amp apart and don't see anything wrong at all. Nothing burnt, discolored, or anything. Both sides of the board look brand new. What should I look for to try to figure out what's wrong with it. Any ideas?

I was hoping someone could tell me what I should check that would cause that. It was working fine at one point. I'd like to think I'd notice a burn spot or something indication where the problem is.

use an Aux to RCA splitter and test the signal output directly from your phone to the amp, if it reads higher, its the head unit, if it reads lower, then its the amp. Same issue with a lot of the vfl 4480.1 hybrids.

 
I installed it in my ride and had the same problems, accomplished the same thing. Anyways, I know the amp is the problem, but want to know what the culprit would be and if its something I can fix for him or if he's better off buying a new amp
