Battery issue

I was at the hospital today (6hrs) and i think i left my lights on, I came out to the car not starting. I have a basic front battery and a kenitic in the back, and when i turn the key i herd clicking and my light in the car were still comming on, i check the front and back battery with a dmm the front was 12.5 and the back was 12.7, i was able to get a jump from hospital police car started right away after jump[, drove home (15 min drive) turned the car and lights off, tried to restart it and the same thing happen car wont start and makening clicking noises, when i put my dome light on and tried to start the car the lights dim. I herd that mean a dead battery. WHAT should i do, get anew battery??? battery is stock and 5 yrs old

 
im going to head out soon to get a new battery. the lights coming on my alarm works, and the battery still hold a pretty nice voltage is throwing me off a tad. deff sounds like a batt. any one eles think it could b the starter ?
