Fuse Holders?????

OK I know this is going to sound stupid but can anyone tell me how fuse holder that accepts either a 0 or a 4 gauge works? I want to use it for a 4 but may later upgrade to the 0. Would this have a an insert or adapter to go to a 4 or is it just a loose fit for the 4 gauge.

every one ive seen has had an adapter that you can take out if you're using 0
