I would say real. Judging by the motion look on the tail and upper body and the way the deer is (head and feet) it looks like it was jumping forwards (they're in my yard everyday and that looks like when they jump forward when you run at them hard). The head isn't cockeyed or limp which leads me to believe that it isn't laying down. The ear also looks like it is in the process of jumping forwards. Also the blood splatter doesn't look like it is necessarily on the deer. It looks like it's a splash still in the air. The blood at the exit wound also looks that way to me. My opinion is real but it also wouldn't surprise me if I was wrong.
