They take a Q-tip and stick it far up there , imagine a dry qtip shoved up there and moved around for about 20-30 seconds, its the most painfull thing i've ever experienced in my life.

Actually it's not a needle per se, more like a 1/4" stainless steel probe. It stretches your urethra in ways it wasn't meant to be stretched.
I didn't even get a local anestetic. Excruciating pain.
