Amp Blown? / What to do

I have a Lanzar Opti 2000d and yesterday it cut out and wasn't nearly as loud. I thought one of the wires to the coils of my sub had come loose, so today I checked it and they were fine. I then checked the amp with my multimeter and the voltage from the speaker outputs only read about 13 volts with the car on and about 12 volts with the car off. This will go down if I turn the gain down but if I turn it up it is about 13 volts is the max. I assume this is the voltage the amp is getting and it isn't amplifying. Is the amp blown? If so how can I fix it, or should I just get another amp. thanks

 
The alternator voltage has always been a bit low, i've had the amp for 6 months and never had a problem before. I opened the amp up and I don't see anything noticibly wrong with it

 
today I messed with the amp some more. I hooked it up to my sub and turned up the volume, it would get loud for a split second and then the volume would drop, every once in a while it would get loud for a split second and go back the the lower volume. The max gain I could get was 26v ac, which is around 1400 watts. The amp is acting really weird.
