I got a Xm unit from my brother and It works on my Radio, but its not SAT. Ready so Why does it work? Sometimes it wont turn on, I'll have to turn my car off and on again for it to work.

Nop, I just hooked it up activated it and it works fine.

Is it a Roady or something similar run through an FM modulator, or is it a receiver which is actually controlled through the head unit?

Its plugged into the CD changer port.
