You're real close to what I sketched up (on paper) for mine. Mine will hinge and slide at the wall so I can fold it up out of the way.instead of building a table i screwed a 2x4 to my wall to let mdf ride on ... its the same height as my table saw... just did this yesterday when i hung my shelves.. works pretty well.. im going to sand it down to make it super smooth.. may even through a piece of plexi over it

Not sure. I bought mine from ebay. I know Grainger sells them but they are like 3 bucks a piece but you don't need many at all. One every square foot should suffice.those bearings look like they would help a lot...can you get them at home depot or other building stores?
