My main qualm with even contemplating getting a PA would be that you probably won't be able to piss straight (the "showerhead" effect); so, get used to sitting down when you need to go. Second, it usually requires a cosmetic surgeon to close up the hole if decide you don't want it anymore. As someone who has had his PA to 4ga, lemme inform you of this. The initial piercing isn't through much tissue, just a thin membrane on the bottom. Infections aren't a problem, they are cleaned 'naturally' daily. When the piercing loosens a little and you go up in gauge it isn't bad if you just go up 1 size. The long term effects some people have, and I think started to happen to me is when the hole gets larger which way does it travel? I felt like mine was traveling to 'join' the existing hole, so I removed it. Three years later, I still have to hold my finger over the F&*^$ing hole!!!!

Some girls loved it, others not so much. Decide how YOU feel about it, you have to live with it.

i have gottin a prince albert twice now (got a gf after the first time who didn't like it and i took it out so it healed up) as for pain it didn't hurt at all when they did it but after words taking a piss stings a little until it healed up. my advice for the first week take a piss often it will help with the pain and sterilize it. i will not take it out and leave it out ever again. i love mine!
