well make sure you can go to their office, in pa you cant just go to the office you have to do it all by mail and phone.

It is on you to prove you made over 5k in the base period. So start compiling all those stubs that I know you saved. If you didn't save them, then you will have to seeif the employer will provide copies of them, but I wouldn't expect them to jump. Otherwise kiss the bennies good bye.
