Sand the paint off with a DA, tape your car off really well and get a can of por-15 and put it on the rusty spots with a foam brush.

http://www.por15.com

Get a small tin of that and put it back over the rust..when it is still sticky get a good grade of U-pol catalyzed primer, you must shoot it when the por15 is still a little tacky otherwise you would never get the primer to stick..

After that, sand the whole panel, another coat of primer, knock it back down with 400 and 600 grit and paint back to original color...

knock off the loose stuff and hit it with a rust converter. the converter will turn it into a black iron oxide and then all you have to do is paint over it:D
ya when i went to order my paint color online it had a rust converter i can buy. thanks./

Do not use POR15 on the body, use on the frame and other suspension parts, but for ur minor rust problems knock off the off paint with a DA 80grit and see if the rust is all the way though if not, make sure u clean it out and then use 180grit (DA) and feather edge around ur area (like 5-7inch) and tape prime and wet sand and paint/ blend ur panels...PM me if got any Questions

just sand with a 220 grit sandpaper and use a self etching or regular sandable primer from autozone or advance auto...shouldnt come back...thats all i've done and i do paint on my own cars and my roomate and i do customer's all the time...
