It is for 1 single 12 and it fires up. It takes up the passenger side. Not sure of the model number of the 12 Let me see if I can go get a pic real quick.

The box has a textured SEM Coat on it. It will need to be repainted but you can buy a can of SEM coat at Car Quest.
