The Subwoofer is a polk dxi 124 dvc connected to an RE audio 2000.1 amp in a 1991 Honda Civic LX model. The amp's rms at 4 ohm is stated to be 410W and the subs rms is stated to be 360W (180 per coil). The gain on the amp is set to about 1/3 and the bass level on the amp is set to about 3db (setting goes from 0-18). Also, when the bass level is turned to 0, the issue still occurs yet there is very little bass. The sub is also in the stock ported box.

It's difficult to see in the video, but whenever it makes that bad sound, it almost looks like it is holding the cone in rather than letting it go out (if that makes sense). Also, whenever that happens the blue LED on the remote flickers a lot so I'm wondering if it's a power issue.

Any ideas?
