What causes a power supply to fail?

You can have all the amperage in the world and still have a low voltage scenerio. Sometimes FET's can have damage from previous low voltage instances and not actually fail until later. Being that this amp is used, that might be a rational conclusion.

hmm okay. I mean there was no way i fed it low voltage. I had nothing wired into the speaker terminals, so there wasnt anything to cause my voltage to drop

You just never know what you are buying when you purchased used, its just part of the game.

 
What causes a power supply to fail?

A few things. The most common being a shorted power supply rail. The secondary side filter caps can fail short to ground. Also output transistors can short causing a constant on condition. This can cook the speaker, other output transistors, and the power supply section.

Another common mistake is letting the amp run too hot for prolonged periods of time. Perhaps improper ventillation? The hotter a transistor runs, the shorter its life expectancy.
