Regular cab Truck setup

I have focal utopia components in my Dodge regular cab... mids in the doors, tweets molded in pillars firing sideways at each other. I also have a 10w7 for lows.

Right now I dont have anything for rear fill. Wanted opinions on whether I need rear fill or not. Will it sound better with some 5 1/4" coaxials? should I amp them?

I know theres not much room in regular cabs so I was wondering if it would make a difference considering the size of the interior.

 
I was wondering how this affected a DVD's audio playback. Some DVD's only play in 5.1 surround. Does that mean Im losing some of the sound since Im only running TWO front speakers?
