Turns out My friend did not need this part in his car. We hooked it up once, and realized that it wasnt going to work in his car and took it out. Comes with everything LNIB, as pictured below.

Connect the input pigtail leads on the XR4 to the speaker level outputs of your existing factory or after-market headunit, and, using the RCA output pigtail of the XR4, plug in the RCA cables from your new amplifier to the XR4, adjust the gain and you are on your way to noise-free, high fidelity sound.
