On a side note, I would re-adjust those settings. You're cutting off a large part of your frequency range.LPF is just above the 50 mark...so prolly around 55HZ....and HPF is just below 33Hz......i set this using the tutorial on the website using my box tuning of 32 Hz as the reference.

HPF - around 27hz
LPF - around 80hz
Start there and adjust until it blends well with your front stage.
About the original question. If you are 100% positive that everything was setup correctly, they could have just been bad amps. I know the odds of that are low, but it is possible. If you blow one more, I would 100% say it's the installation.
He's running an Orion 2500D, class D mono......as long as they are wired together properly then he should be fine.are your subs bridged to the amp?? thatll cause it to blow, maybe your subs wires touched. Petty strange indeed.
