The PVC flares you see were done by making a form for whatever size port we were making. Then we heated a big *** pot of cooking oil to 350*. Soak the PVC in that oil for a minute or so and it gets pretty buttery. Push it down onto that form and voila, instant aero. Well, after it cools, of course. It's messy business, but makes some pretty sweet ports.
On that port (it's an 8"), I made one side flared, but pushed down on it a little further to flatten it out essentially making it a mounting flare.
Should be noted that I rarely mount the actual flare to the box. Normally, I make the cutout juuuuust big enough to beat the tube into and then mount the flares on either side. This makes manipulating tuning and/or impedance rise easier because you can just slide it in and out and test how much in and out is the best combo.
