Amp is putting out low voltage

what would make an amp put out low voltage? I have a peavy cs800x amp it runs on standard 120 and when bridged at 4 ohms I should be able to get 1200 watts rms. I bridged it at 4 ohms and only get 23 volts max which is about 125/150 watts approximately. What would make an amp do this? I realize that this is car audio but people who know the guts of an amp might be able to shed some light on this. Any bit helps ...

 
I got it to get a little higher - putting out low 40s - still not the 1000 rms it should put out - but I'll take ever volt I can get. I'm going to use a test time next

Does the DDT light ever come on?

Is the Power light on Channel B off?

When you say max was in the 20v out, what is max? max volume?

If that's true, how did you get it to increase into the 40s?

What is a "test time"?
