Amp fuse melt

HI all,
what can melt an amp fuse? i have install that 3 years ago and yesterday, saw that there was no audio. check the fuse and saw that it melted.
also, what can be done to avoid that?

good questions but have no clue of what they have installed.
The jacket on the wiring should have the Brand along with size and type of wire and some have strand count. By the looks of the Burnt wiring as Ive seen over the years.. to me it looks like CCA wiring by the Charcoal burn color look. Not Sure on the size by the Pick but taking a Wild Guess that its 4gauge??
 
Most melting near connection points are caused by loose clamps (bolts, screws, blade holders, crimps).

Seems the set screws always loosen up and that will cause it to melt.
I've had trouble with those style fuses myself and as you say the bolts always seem to loosen up and cause issues. Some I've used aren't tapped deep enough for the bolt to really cinch tight in the first place and you'd need to use a washer right from the start.
