Every install is different but most people will say sub up port back. However if you don't have a lot of room for the port (aiming back) to get back over the height of the box and back up front sometimes the port up might help.

Well depends on your port, but usually its recommended to have ATLEAST the diameter of your port distance away from the hatch if your using a slot port, and an aero port you can get away with a little less but still. Then I would say atleast 15" from the ceiling atleast but that is just a guesstimate because it depends on the vehicle, the lower the better.

 
Here are a few pictures of mine...and both of them were really to tall/close to the hatch.
