Sony sony xm-404eqx Amp Offset

Hey, this is my first real car audio venture, i got the amp listed above, when its hooked up its great powers great and what not. When i turn the key from off, to either start the van or to turn key backwards and listen to music the Offset light goes red.

If i disconnect the amp from power with the van running or the key already on and re-connect it, it works fine. I have 2 other amps that ive hooked up and none have had this issue, although i do not know if they have offset protection.

Any advice as to how i can fix this?

 
Some more info, i do not have an aftermarket deck, i cut one end of a rca cable and spliced the wires into exhisting speakers, would this cause this to happen. Once again the other 2 amps i have work just fine.

 
this thing bangs nicely when working, tired new ground and more/bigger ground wire, still isuure, could the fact im getting my 12V= through the car's fusebox be causing my issue? Ill try using a different fuse location tomorrow.
