Cold start.

try seafoam if the IAC cleaning doesnt help.

I personally let it go through a vacuum line (like brake booster) then let car sit off for 10-15 minutes and start it up. It will run rough but it gets rid of all the carbon.

and LOTS of smoke! Your neighbors might not like that, unless you could care less about them. lol

 
Cleaning the IAC might work for a few days, but it almost always goes bunk afterwards.

Granted I had luck cleaning mine and it solved the exact same issue and its still going *knock on wood*

Car-Duratec(24v V6) Taurus.
