The state uses those grants for any number of programs, from healthcare, to schools, road repair, other infrastructure, law enforcement, public assistance programs such as TANF, SNAP, Medicaid, etc.
Regardless of what the grants are used for, any state that gets back more than it gives is a welfare state. A “taker”. The money they get comes from the “giver” states.

The irony being the blue states generally support more taxation, higher tax rates, etc that are detrimental to them and beneficial to the red states who rail against the very policies that they benefit from.
