9004 model, I bought them, they were the wrong model (dumb*ss Taurus manual told me wrong) and now I'm selling them.

One of them has some minor scrapes on one of the notches because I started to try and make them fit, but I gave up. Its nothing that will affect the fit at all. The bulbs are both in brand new condition.
