its an oem for amp pulled from a '95 windstar. i dont have the model number offhand and there really is no info about it online other than 1 or 2 sites selling it (for $200+. i made out and got it for $25 shipped off ebay ). theres actually 2 plugs on it. the one posted above and another...

pins 4, 5, and 6 are there on the amp, but im guessing they just arent used. im as confused as you are on that one anyways, i did come up with another site that, although for another oem amp, may just answer the question. still all speculation here.
