I emailed USA Spec and they said its probably a faulty logic and control circuit in the adaptor. I will be installing another one and hoping for the best.

I have a 2004 Accord LX with a stock 6 disc CD changer and no navigation. Over the weekend I installed an iPod connector (PA11-HON2y from USA Spec); it's an old model but it was new in the box. I'm not using the iPod cable, just the RCA out, which plugs into a blue tooth device (Kinivo BTC450)...
